CAMPER  Registration

All steps of this registration process must be completed by July 1st to secure a spot.
Camper spaces are limited, and are available on a first-come-first-serve basis.

Step 1: Register Your Camper(s)

The following must be submitted (along with the medical documents under Step 2 below) by  July 1st  to ensure placement.

Step 2: Submit Medical Documents (2 forms & 2 documents- 4 total submissions)

​

The following must be submitted for each camper by July 1st to ensure placement.

 

  • Click the links for  TWO FORMS below for fillable PDF forms:​

FORM #1- Health History Form (required every year)

FORM #2- Physician’s Consent Form​ (required every two years)

 

  • Submit these forms by:

  • Additionally, please you will need the TWO DOCUMENTS below:​

DOCUMENT #1-  Immunization Records 

DOCUMENT #2-  Medical Insurance Card

Given some recent changes to PayPal, we are temporarily only able to accept payment by check or money order. We apologize for the inconvenience, and greatly appreciate your effort and support!
